Joseph (Reading, PA) JOB SUMMARY: The RN Registered Nurse Navigator is responsible for coordinating the individualized care continuum for patients diagnosed with, and receiving care for, cardiovascular conditions, including heart failure and stroke, and for achieving quality, cost-effective and patient-focused outcomes by serving as an advocate, educator, consultant, leader, and community ambassador for this specific patient population in accordance with hospital standards/guidelines, applicable statutory requirements, and accepted clinical practice for the clinical nurse specialist profession.

An incumbent is a consistent care coordinator throughout the continuum of care and is able to assess the physical, psychological,and social needs of the patient.

Work is strongly focused on: 1) guiding the patient/family through the complex healthcare system, working to eliminate barriers that might otherwise adversely impact patient care/outcomes; and 2) helping to coordinate the efforts of the medical team to facilitate continuity of care and enhance patient satisfaction.

Work requires strong cardiovascular nursing expertise and an understanding of navigation concepts, patient care standards/practices and applicable regulatory requirements specific to the assigned specialty area.

An incumbent exercises a high degree of discretion/independent judgment and applies advanced skills obtained through extensive clinical preparation to ease the burden on patients/families while improving patient outcomes through monitoring and education.

Situations encountered entail the application of sound critical thinking skills in the face of highly stressful situations impacting the patient/family and involving difficult decision-making.

Education: Bachelor’s degree in Nursing (BSN) from an accredited college or university.

Master’s degree in Nursing or related specialty certification is strongly preferred.

Experience: Five years of related professional nursing work experience.

Previous cardiovascular navigator work experience is strongly preferred.

License/Certification: Current RN licensure Current BLS Certification.
