Job Summary The Resident Care Coordinator will manage, supervise, and coordinate the care of residents, and will assist Long-Term Care DON of day-to-day operations of nursing home and act as a resource to staff by performing the following duties: Key Responsibilities
• Assist MDS Coordinator in the development of resident care plans, and ensure the update staff group sheets and communicate changes.

• Evaluate activities of each neighborhood to ensure patient care, staff relations, and efficiency of service.

• Evaluates to ensure standards of care and Federal/State regulations are followed.

• Attends meetings and acts as a back-up to the MDS Coordinator for care conferences as required.

• Assists in training, disciplining and developing staff.

Assists in the CNA evaluation process as delegated by DON.

• Assists in developing, monitoring and evaluating quality and appropriateness of nursing care being delivered.

• Attends morning report.

• Review resident incident reports and educates staff.

• Responsible for resident admission, transfer, discharge for neighborhood.

• Acts as a liaison between resident, family member and practitioner to facilitate health needs/concerns.

Communicates and documents events appropriately.

• Other duties as assigned Additional requirements for this role include:
• Current Registered Nurse (RN) license that allows you to practice in our state of Minnesota.

• Graduate of an accredited nursing program preferred.

• 2-3 years experience.

• Basic Life Support (BLS) required
