Registered nurse (rn) nurse navigator at north star lodge

We are looking for an RN Nurse Navigator to join the team at Memorial’s North Star Lodge – where world-class technology and innovations in cancer care are brought together in a beautiful environment specifically designed for healing right here in the Yakima Valley.

ROLE SUMMARY The RN Nurse Navigator navigates patient and family members through diagnosis, treatment and follow-up to ensure a smooth transition between care settings.

Collaborates with a multidisciplinary team to support patients and families in addressing necessary education, psychosocial, advocacy, and continuity of care needs.

ESSENTIAL RESPONSIBILITIES include: * Provides continuity of care by ensuring smooth transitions between care settings.

Develops relationships with patients and families and with NSL’s multidisciplinary teams to facilitate/navigate treatment and follow-up care.

* Completes a comprehensive nursing assessment to identify the patient’s behavioral, psychosocial and physical individualized needs; collaborates with the multidisciplinary team to develop a plan of care.

* Supports patients and families by providing education, addressing behavioral and/or psychosocial needs, and identifying barriers to care; advocates as necessary to ensure continuity of care.

* Assists leadership in developing program vision, goals, and objectives.

Assists in the development, implementation and maintenance of quality programs that include best practice for patient care outcomes including quality of life, and enhanced patient loyalty.

QUALIFICATIONS AND REQUIREMENTS Education: Associate’s degree in Nursing required; BSN preferred.

Experience: One year of Oncology experience required; three years’ of Oncology experience preferred.

Licenses/Certificates/Registration: Oncology Certified Nurse preferred and required within one year of hire.
