Population Health Specialist/Case Coordinator (Non-Clinical)

This is a Contract position in Fisher Island, FL posted May 22, 2019.

Description: The Population Health Specialist/Case Coordinator will work with medically complex individuals, many who may have one or more chronic disease conditions, and social barriers.

He/she identifies the unmet needs of patients and assists them with accessing care, and links them to community services and resources.

The Population Health specialist will support a primary care model of care, collaborate with the health care team across the health system and community, foster and engage patients in their own care and address social determinants of health.

Through the gathering and review of medical data, and information provided by the patient, the Population Health Specialist develops and prioritizes the patient’s needs, sets mutually agreed upon goals with the patient and develops a service plan outlining interventions and target dates for achievement.

Qualification: Experience: Required: 2
– 5 years of relevant experience within a community health setting managing populations with complex medical and/or behavioral health needs.

Prior experience with electronic documentation, and experience documenting in multiple computer applications.

Prior experience and working knowledge of Medicare, Medicaid, managed care and commercial payor principles, and benefit coverage and limitations.

Prior experience working with uninsured populations.

Desired: Prior experience documenting in an electronic medical record (EMR).

Prior Case Management experience.

Completion of AHCA approved mental health targeted case management training.Education: Bachelor degree from an accredited university in a health or human service area of study.Licensure Certification: Bilingual English/Spanish or English/Creole fluency.

Able to speak, read and write in both languages without limitations or assistance.

Knowledge of local community health and social service agencies, programs and resources, and experience arranging community events.

Current Case Management Certification (CCM) and/or Social work licensure are a plus.Knowledge Skill Abilities: Bilingual English/Spanish or English/Creole fluency.

Possess intermediate to advanced computer skills and experience with Microsoft Word, Excel and Outlook.

Knowledge of state and federal statutes, rules and policies that affect the targeted populations.

Use of private vehicle for company business.

Valid state driver’s license and proof of personal vehicle liability insurance with limits set by the organization.
