Physical Therapist for Adult Day Care Center

This is a Contract position in Jacksonville, FL posted August 5, 2021.

The Adult Day Care Centers located on our Manhattan campus and Waters Place, Bronx are seeking full time Physical Therapist to join our team.

Job Summary: Qualified candidate will provide physical therapy services consisting of daily assessment and treatment of clients in the Adult Day Center program including completion of all necessary documentation.

Duties and responsibilities: 1.Provides PT evaluation of physical and functional status as ordered by MD.

2.

Provides PT treatment to restore functions, relieve acute and chronic pain and to maintain optimal level of independence.

3.

Provide initial screening of all ADHCP registrants and annual screening thereafter.

4.

Select, order and maintain rehabilitation equipment for registrants.

5.

Provide instruction to registrant, HA and family in use of equipment ordered.

6.

Completes all documentation and communicates findings to MD when indicated.

7.

Refers registrants when indicated to prosthetist, orthotist, shoe clinic, nursing rehab, ambulation program and follows up on subsequent orders.

8.

Communicates and collaborates with other health care team members including MD, OT,ST, Rec, SW, Nursing as needed in care plan meetings.

9.

Provides inservice to staff and home attendants.

10.Provide home evaluation and community ambulation evaluation when indicated.

11.Create quality assurance specific to PT needs of registrants.

12.Evaluate toileting and ambulation assistance needs of registrants and provides Written instruction for staff use, in the form of a list which is updated quarterly.

13.

Participates in staff meetings, team meetings, care plan meetings, registrant review and in mandatory inservice education.

14.Maintains professional competence through participation in continuing education and other appropriate learning experiences.

15.

Provides monthly statistics on PT.

16.

Supervisor PTA and co-signs notes when necessary.

17.

Any other related duties as required Denotes essential job function.

SPECIALIZED SKILLS AND COMPETENCIES: 1.

Ability to evaluate physical and functional status and assess potential safety hazards in the community, facility and home.

2.

Ability to communicate findings and recommendations with members of health care team, or family and registrants.

3.

Ability to set realistic goals and implement appropriate treatment plans in accordance with current Physical Therapy standards.

AGE RELATED SKILLS AND COMPETENCIES: 1.

Ability to relate to and understand the needs of the elderly.

2.

Understanding of special needs, abilities and behaviors of registrant with dementia.

PHYSICAL DEMANDS: 1.

Transferring registrants from sitting to standing positions, from wheelchair to chair, toilet, or bed and back.

2.

Pushing wheelchair, bending, lifting.

3.

Assisting with PT at equipment (11 bars, etc.) 4.

Performing home visit and community ambulatory assessment.

JOB QUALIFICATIONS: 1.

BA/BS in PT from an accredited curriculum.

2.

Valid NYS PT license required.

3.

Minimum of one year of experience working with geriatric population strongly preferred.

Ability to relate to and understand the needs of the elderly.

Understanding of special needs, abilities and behaviors of registrant with dementia.
