Everything we do centers on one goal: improving patient care Position Responsibilities: Collaborate with physician practices and members of the healthcare team to manage a complex caseload while promoting the effective utilization of services and care coordination for adult, geriatric, neonate, pediatric and adolescent patients Assess each person’s health care needs, monitor effectiveness, and progress plans to achieve desired outcomes; coordinate social services to address psychosocial needs.

Utilize expertise to review plans of care, identify barriers to effective and efficient utilization of resources and evaluate patients’ needs throughout the continuum of care.

Actively participate in and initiate efforts to enhance patient flow and decrease length of stay, while increasing patient satisfaction and reducing readmissions Collaborate with inpatient staff and physician groups to determine goals for level of care, promote length of stay management and coordinate post-acute transitions.

Confer, negotiate and advocate with physician practice groups, community providers, patients and the health care team regarding progression in the plan of care and dealing with variances.

Provide education for staff including care managers, liaisons, nurses, physicians and allied health professionals.

Model and maintain a quality based proactive person-centered approach to achieve department and institutional goals and process improvements.

Position Qualifications Include: BSN required.

Master’s degree in Nursing or related health discipline preferred Current RN License Three (3) or more years’ experience as a Registered Nurse, Nurse Practitioner, Nurse Case Management Ability to serve as a team resource for clinically complex cases within professional (nursing) expertise Excellent interpersonal skills and evidence of the ability to collaborate effectively with physicians, nurses, and members of the healthcare team.

Ability to travel throughout the Medical Center.

Case Management certification (CCM) preferred (or commitment to obtain within 2.5 years) Current CPR Certification preferred.
