This is a Full Time position 80 hours every two weeks, benefit-eligible Position Highlights: The Manager of Patient Care Navigation is responsible for daily operations of the Population Health patient care navigation program.

This role is responsible for successfully managing program quality standards, performance improvement, and personnel performance and productivity.

The Manager of Patient Care Navigation is responsible for the overall planning, organizing, and oversight of key navigator activities.

The Manager is responsible for building and maintaining positive team dynamics and addressing any conflicts should they arise.

The individual in this position will collect and report on the patient care navigators¡¦ performance data and will establish a data infrastructure for continuous performance improvement.

The Manager of Patient Care Navigation will align program goals with those of the Population Health Department.

The individual who holds this position exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

Promotes and educates others across the organization on the impact of the patient care navigators.

People Management Recruits, selects, employs, orients, and terminates the employment of personnel within the program Motivates staff to function as a team in achieving goals and maximum productivity.

Encourages and nurtures navigator creativity and innovation.

Promotes a work environment that includes open communication, collaboration, service excellence and a teach approach.

Manages diversity and supports equal and fair treatment and opportunity for all.

Builds effective teams and fosters open dialogue.

Evaluates the performance of employees providing direction and meaningful goals on a consistent basis; recognizes positive employee performance; administers the disciplinary action process; maintains accurate manager¡¦s counseling notes.

Conducts annual performance evaluations.

Monitors navigators¡¦ performance in accordance with departmental policies and procedures; ensures department quality standards are consistently achieved.

Manages all employee time off requests and coordinates adequate staffing coverage.

Supervises daily work and outcomes.

Coordinates office resources in conjunction with the Director.

Data and Report Management Is able to prepare, implement, measure, and report quality improvement initiatives Manages individual team member performance dashboards and effectively integrates data into coaching conversations.

Monitors the quality of the data and information contained in reports.

Identifies trends found in the data and communicates findings to Population Health leadership.

Utilizes analytical skills for significant data interpretation related to operations.

Position Qualifications Include: Masters degree required Leadership and management skills necessary to plan, organize, direct, and control the activities of personnel.

Service/quality mentality and attitude.

Effective organizational and time management skills.

Demonstrated ability in problem solving.

Effective verbal and written communication skills and interpersonal skills; ability to communicate with all levels of the organization.

Ability to work independently, to prioritize, meet deadlines, participate in improvement projects or teams.

Excellent interpersonal skills necessary to interact, motivate, and communicate with a wide range of staff in a professional manner Strong analytics, organization, and problem-solving skills.

Minimum 5 years of experience in the health care field.

Minimum 2 years of management experience, including staff motivation, required Demonstrated proficiency in the use of Microsoft Office Suite; proficiency with data management software.

Strong desire to develop staff.

Physical Demands: Ability to travel throughout the Medical Center and off site for scheduled meetings.
