Licensed Practical Nurse

This is a Contract position in Osage County, OK posted July 23, 2019.

Job Description: Under direct supervision of RN Case Manager, coordinate and implement the plan of care for each hospice patient on an as-needed basis.

Plan of CareGive and demonstrate safe, individualized nursing care to selected patients and families according to patient plan of care.Continue family/patient instruction initiated by the RN Case Manager.Perform selected nursing procedures as assigned by the RN Case Manager according to Hospice standards of practice.

Documentation​Record observations and treatments/care provided completely, accurately, legibly and concisely according to documentation procedures.Report any changes in the condition of the patient or family situation to the RN Case Manager.

Case ManagementPrepare, administer, order and record medications.Participate in IDT meetings at the request of the RN Case Manager.Make necessary changes in the work plan to compensate for emergency situations and/or acutely ill patients.Report significant changes in patient condition and/or treatments to the RN Case Manager as they occur.Consult RN Case Manager immediately if a patient situation is urgent.Share pertinent information with appropriate staff personnel and other health team members.

EducationParticipate in mandatory in-services.

Other DutiesPerform any other duties assigned by supervisor.

Qualifications:LPN license in good standing from State Board of Nursing.Graduate of LPN program from a state-accredited institutionOne or more years of current professional hospice nursing, home health or acute care, oncology or radiation oncology experience preferred.Must possess communication skills sufficient to interact effectively with the patient and their families, as well as all interdisciplinary caregivers, peers, subordinates, and supervisors.Must be willing to work any assigned schedule of duty as needed.Requires reliable transportation, current driver’s license and current auto insurance.Must have useable experience using Kinnser software or be prepared to receive training Requirements:Must maintain complete confidentiality of patient records, all other confidential information received during the course of patient care (HIPAA).Must comply with all hospice policies and professional standards.
