This is a Full-time position in Gambrills, MD posted October 8, 2021.

Position will cover Anne Arundel County # # Job Summary Provides personal care for the Agency#s patients.

Minimum Qualifications Education/Training Ability to read, write, and perform arithmetic calculations; high school graduation or equivalent preferred.

# Experience 6 months home care experience preferred.

# License/Certification/Registration CNA (Certified Nursing Assistant), GNA (Geriatric Nursing Assistant), or HHA (HomeHealth Aide) certification in the District of Columbia, Maryland or Virginia, or any combination as required based on work location(s).

CPR certification required.

Completion of MedStar Health#s home health aide wound, diabetes and CHF competencies after hire.

# Knowledge, Skills # Abilities Verbal and written communication skills.

Basic computer skills preferred.

Primary Duties and Responsibilities Attends Agency regional office and team meetings as required.

Attends in-service educational programs.

Demonstrates flexibility in adjusting to changes in changes with procedures and in daily schedule.

Cleans direct patient care area as necessary.

Identifies safety hazards in the home and reports it to the team#s Operations Director or primary case-manager.

Reinforces with patient/family ways to remove identified safety hazards in the home.

Contributes to the achievement of established goals and objectives and adheres to department policies, procedures, quality standards, and safety standards.

Completes annual Code of Conduct, Compliance and other mandatory training.

Complies with governmental and accreditation regulations.

Documents services in accordance with Agency medical records and timekeeping policies, including the absence of patients from their homes.

Documents patient progress reports clearly and concisely for multidisciplinary case conference.

Maintains effective working relationships with other departments and participates in multidisciplinary quality and service improvement teams.

Participates in meetings and on committees and represents the department and agency in community outreach efforts.

Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.

Performs other duties as assigned.

Performs personal care activities (bathing, dressing, oral hygiene etc.).

Performs other related activities (light meal preparation, personal laundry etc.).

Assists with transfers and range of motion exercise programs as instructed by professional staff.

Performs procedures as delineated on HHA Skills Competency Checklist as instructed by professional staff.

Recognizes signs of emotional distress (such as anger and depression).

Encourages patient mobility and socialization during visits.

Recognizes and responds to special needs of patients/families experiencing terminal illness.

Reports identified alterations in patient behavior or mental status to primary case manager or the team#s Operations Director.

Reports identified patient care or family health or social problems to the team#s Operations Director or primary case manager.

Reports all problems in carrying out assigned activities to primary case manager or the team#s Operations Director.

Collaborates with case manager or professional staff in the revision and implementation of the Home Health Aide plan of care.

Informs the team#s Operations Director of need to change patient#s visit schedule.
