Heart failure nurse navigator inpatient – austell

Responsibilities The Heart Failure Nurse Navigator Inpatient is a registered professional nurse with advanced clinical expertise whofunctions under the direction of the primary Heart Failure Destination Unit Nurse Manager.

Role responsibilities includethe development, implementation and maintenance of the heart failure program including assessment and treatmentof heart failure patients, development of clinical practice guidelines to optimize patient outcomes, collection andanalysis of heart failure data, monitoring compliance with heart failure guidelines and program initiatives, performanceimprovement, staff education, and ensuring compliance with regulatory requirements.

The RN Heart Failure Navigatorserves as a clinical role model, educator, and liaison for the heart failure team, hospital staff and community andcollaborates with the medical director and nursing leadership to facilitate and promote improved patient outcomes.

Qualifications Required Minimum Education:
• Graduated from an accredited school of nursing with a Bachelor’s Degree in Nursing.
• Master’s Degree in Nursing preferred Required Minimum Certification:
• Currently licensed as a Registered Nurse in the State of Georgia or hold a privilege to practice in the Stateof Georgia under the Enhanced Nurse Licensure Compact (eNLC).
• National Heart Failure certification preferred Required Minimum Experience:
• Minimum two (2) years recent cardiology clinical experience with demonstrated teaching experience in theclinical and community setting Required Minimum Skills:
• Strong clinical background in an acute care setting
• Strong customer focus
• Excellent written and verbal communication skills
• Proficiency in use of electronic medical records
• Ability to organize and lead meetings as well as present in groups
• Ability to work independently, delegate, and multi-task in a fast-paced environment
