Our UT Health Athens hospital is seeking an experienced Clinical Manager .

We are looking for your experience, knowledge and patient care skills in our Home Health where you will be responsible for: What You Will Be Doing: Initial and ongoing comprehensive assessments of the clients needs, including Oasis assessments at appropriate time points.

Initial and revise POC as necessary.

Plan discharge Coordinates all care required for client referrals assigned to them.

Caseload levels to be established by Agency Director with the guidance from Corporate Director of Home Health Makes personnel and patient assignments as needed Accurately documents all activities Assures the development, implementation, and updates of the individualized plan of care.

Participates in the Performance Improvement activities of the Agency Maintains contact with other Case Managers, Agency Clinical Directors, physicians, client, families, and other concerned persons.

Informs physician and other staff of changes in client needs Maintains current knowledge in the field through attendance at continuing education and in-service programs Provide services and treatments requiring substantial and specialized nursing skill.

Counsel and educate client and family regarding disease process, self-care techniques, preventions, strategies, health promotion and disease prevention.

Promote client and family independence.

Initial appropriate preventive and rehabilitative nursing procedure Supervise all skilled nursing services and home health aides assigned to their Care Team by monitoring and evaluating skills and performance Details: So what experience are we looking for in our Clinical Managers?

Registered Nurse with current Texas license to practice, BSN preferred One year of Home Health experience preferred Six months to one year Case Management experience preferred Good clinical skills, judgment, and analytical capabilities Current CPR certification.

Excellent human relations Supervisory experience Excellent communication skills, clinical skills, judgment, and analytical capabilities Some computer operation knowledge Some knowledge of Microsoft Windows Excellent coordination and evaluation of client care Knowledge of Medicare/Medicaid Home Care Standards Now that you read about what you will be doing and the experience we seek, it is time to apply and make a difference in the lives of our patients.
