Clinical Health Educator – Patient Education

This is a Full-time position in Roseville, CA posted June 13, 2019.

Position Overview: POSITION SUMMARY:Th Clinical Health Educator provides medical nutritional therapy (MNT) and Diabetes Self Management Education (DSME) to patients with diabetes, pre diabetes, disease processes related to cholesterol, renal, and endocrine disorders.

The Clinical Health Educator also works collaboratively with the treating physician to manage and monitor treatment and the response to therapy.The educator also provides ongoing education and consultation to nursing and other ambulatory staff, supports and provides physician education, and supports clinicians in hospital and other patient care environments.This position also providesnutritional care (MNT) and counseling to adults and children on clinician referral, collaborates with the health care team.

Qualifications: MINIMUM POSITION REQUIREMENTS: Education: Graduate of an accredited School of Nutrition/ Dietetics or Nursing.

Current registration with the CDR as an RD, or California license as an RN.

Current CDE Certification strongly preferred.

Attendance at an approved conference or accumulation of a minimum of 15 ADA approved CEUs per year.

Maintains or will obtain current IRB training for potential research projects.

Experience: Experience required in care and management of diabetes and other related disorders.

Experienced in medical nutritional therapy across all topics is strongly preferred.

Completion and maintenance of CDE certification implies at least 1000 hours of direct care experience, or actively working towards certification as a CDE.

Previous experience in working with small and large groups, learners across all age groups, and multidisciplinary teams is preferred.

Experience in research is beneficial, IRB training required.

Knowledge: Contemporary knowledge of all aspects and options in the care and management of diabetes is required.

Understanding of the adult learning model and group dynamics is required.

Ability to work with a team to develop collaborative educational approaches and curriculum.

Strong computer skills are required and the ability to adapt and use the electronic medical record is required.

Must have strong clinical assessment skills that support autonomous practice in the ambulatory setting.

Must have excellent problem solving skills and critical thinking skills to make recommendations to providers for medical management of diabetes as well as other disease processes.

Knowledge of the research process and IRB is preferred.

Special Skills/Equipment: Excellent oral and written communication skills.

Ability to conduct classroom activities including using Audio visual aides.

Must be able to travel to various treatment and classroom locations and work with multiple teams.

Excellent customer service skills required.

Ability to problem solve issues quickly and work in a variety of settings.

Excellent time management skills required.
