Clinical Case Manager – Transitional Housing

Jubilee Village, one of Outreach’s transitional housing programs, partners with young single mothers ages 18-26 years old and provides supportive housing, case management services, clinical services, and mentorship to help residents reach their goals, develop as leaders, launch into independent living, and embrace a new narrative for their story.
Job Responsibilities:
The Clinical Case Manager strategically supports the residents of Jubilee Village, as they journey towards their goals and self-sufficiency by providing case management as part of a collaborative team made up of professional staff and volunteers. Responsibilities include managing a caseload of 8-10 therapeutic casework clients, working with clients to set goals related to employment, making and adhering to a budget, completing schooling through high school or beyond, seeking counseling for mental health needs, finding appropriate child care, develop parenting skills and life skills, leadership development, and providing necessary transportation. The Clinical Case Manager will connect residents to resources and advocate for residents with other service providers. The position reports to the Jubilee Village Program Coordinator and participates as a member of the broader transitional housing team.
Requirements:
MA/ MSW with at least 1 year of case management experience, or BA/BSW with 3-5 years of experience in case management. Demonstrated success as a case manager; experience in working with marginalized populations; administrative experience; a passion for guiding young women to envision a successful future; ability to develop effective relationships with participants and outside service providers; resilient and active listener; attention to detail; self-starter and initiator; ability to work independently, as well as collaboratively with a team. IL Driver’s license, reliable transportation, and safe driving are required. Proficiency in MS Excel/ Outlook/ Word. A commitment to Outreach’s Statement of Mission & Faith. Bilingual skills (Spanish & English) are a plus.
Status:
Full-time. Monday – Friday, 10:00 am to 6:30 pm. Two to three evenings a month for life skills training at team meetings; other occasional evenings or weekends to meet client needs.
Compensation:
The hourly rate falls between $20.20 and $22.80 based on education, skills, and experience. Additional compensation for bilingual ability (Spanish & English).
