Nursing Lv 1 (Nurse Practitioner) Tour II ( 8:00AM-4:00PM)PURPOSE OF POSITION: Under the supervision of the Chief of Ambulatory Care, the Associate Director for Ambulatory Quality and Population Health is responsible for implementation and monitoring of ambulatory care initiatives targeting improved clinical outcomes Team development and leadership Clinical process redesign Education of staff Lead implementation and monitoring of innovations Provide direct primary care to a panel of patients (2 days per week) Lead interdisciplinary improvement teams in a Shared Governance model Develop standard work documents for new processes Train clinical staff in new workflows Develop staff and team engagement around new initiatives Coach individual staff around adopting new work flows as necessary Monitor implementation Review and analyze quality metrics and develop improvement plans Attend departmental, interdepartmental and corporate meetings as assigned Develop educational curricula and provide training for ambulatory care transformation projects Assist in the preparation, coordination, implementation, and evaluation of Training and Education Activities for Nursing Personnel Participates in special projects and performs other duties as indicated Minimum Qualifications Licensed and currently registered to practice as a Professional Nurse in New York State, plus (2) or (3) below.

A Master’s Degree in Nursing Administration, Education or an equivalent field and a minimum of six years of satisfactory nursing experience, of which four years shall have been in a supervisory, teaching or administrative capacity; or, A Baccalaureate Degree in Nursing and seven years of satisfactory nursing experience, five years of which shall have been in a supervisory or teaching capacity; or, A satisfactory equivalent combination of education and experience.

However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State.
